We may accept registrations from patients who live outside of the practice boundary or who move outside of the boundary and wish to remain on our practice list.
There are limitations to the services we can provide to out of area patients, mainly that we can not provide home visits. We do ask that you continue to attend the practice for any reviews when requested.
If you are happy to remain registered under these limitations you will be asked to sign an Out of Area declaration.
We will regularly review the care needs of all patients living outside of our practice boundary. If a patient’s care needs become greater and it is determined that the practice can no longer provide the appropriate level of care, we will arrange a discussion with a GP to explore the possibility of transferring to a practice closer to their home. The GP will clearly explain the reasons for this recommendation, focusing on ensuring that the patient receives the best possible care.
